State rules require that all student athletes have a physical examination before starting athletic practices each year.
The Johnston Booster Club arranges for volunteer physicians to provide screening of athletic physicals as a fundraiser each summer. This will satisfy the requirements for athletic physical exams, but should not be considered as taking the place of an ongoing relationship with your personal physician. Any problems identified will be referred to your personal physician.
Please complete the information on the physical form and bring with you on the day of the Booster Physical screening. Be sure to complete the insurance waiver information also on the back of the physical form.
At your discretion, the physical can be performed by your physician. Have him/her complete the form below and return it to the Activities Office, P.O. Box 10, Johnston, IA 50131.
