Let the Enrollment Process Begin

2017-18 School Registration

We are excited to bring you an advancement in the school registration process. Starting April 28, 2017 students re-enrolling to the district for the 2017-18 school year can register online! No paper packets will be sent to homes as in years past. If you are not a resident, you must apply and meet all deadlines for open enrollment.

Parents wishing to re-enroll their child(ren) in the district should log into their Infinite Campus parent portal and begin the process. Instead of filling out multiple forms for each child, parents will verify the current information on their child and approve it for the upcoming school year. This applies to all children who have been enrolled in the district, including the 4-year-old preschool and developmental kindergarten programs.  If you have a new student to the Johnston Community School District, you can enroll them within the portal link as well.  If you know you are NOT returning to JCSD for 2017-18, please contact your building secretary to let them know.

2016-17 School Registration: Follow these steps to begin your journey with Johnston Schools!

  1. Determine if you live within the district or need to open enroll.
  2. Provide proof of residency.
  3. Provide proof of age.
  4. Provide a Certificate of Immunization.
  5. Print and complete applicable forms from the Enrollment Packet.
  6. Once all forms are complete and you have collected the necessary documents, please send to the enrollment office via email (enrollment@johnston.k12.ia.us) or fax (515-278-6303).
  7. Wait to hear from the school building you are assigned to, generally within 48 hours.

Detailed 2016-17 Registration Steps

  1. Provide proof of residency. All new students entering the district must provide the proper documentation to establish residency in order to enroll. All documents provided must state the student’s physical address, and residency at that address is required. A post office box is NOT an acceptable address for the purpose of determining residency. Please be aware that you may not attend the elementary school in your neighborhood. Elementary students will be placed in an attendance center based on class sizes (Board Policy 501.18).
    1. If you own your own home – proof of residency for homeowners can be verified by the Polk County Assessor’s website (please print the page as verification). If home ownership is too current to be reflected on the website, please provide ONE of the following:
      1. Purchase contract with possession date of closing date;
      2. Most recent property tax bill;
      3. Recent mortgage statement;
      4. Utility bill from the current month and mailed to your in-district residence.
    2. If you rent – please provide ONE of the following:
      1. A current signed lease agreement/rental agreement that must contain property owner’s name, address, and signature
      2. Utility bill from the current month and mailed to your in-district residence;
      3. A pay stub for the current month from your current employee showing in-district address;
      4. Or a notarized letter from the landlord.
    3. If you live with a family already in the district – please bring ALL of the following:
      1. Completed and notarized Co-Resident form;
      2. Proof of your residence at that address (i.e. bank statement, insurance statement, current utility bill or other bill, current pay stub, etc. with your name and address clearly listed and within the last or current month);
      3. Verification of residency for the family with whom you are living, such as their tax bill, mortgage statement, current lease agreement, or current bill.
  2. Provide proof of age. Several options are acceptable:
    1. Hospital birth certificate;
    2. Passport;
    3. Adoption papers;
    4. Certificate obtained from the county the child was born in (for Polk County: Polk County Recorder’s Office, 111 Court Ave., Room 245, Des Moines, Iowa 50309-2251; Phone 515-286-3781);
    5. State certificate (for Iowa: Iowa Vital Records Bureau, Lucas State Office Building, 321 E. 12th, Des Moines, Iowa 50319; Phone 515-281-4944).
  3. Provide a Certificate of Immunization. If you were previously an Iowa resident, these records will be transferred by your previous school. If you were NOT previously an Iowa resident, you will need to provide this proof prior to beginning classes. Contact a local physician or the Polk County Health Department.
  4. Print and complete applicable forms from the Enrollment Packet. A legal parent or guardian must complete all the following forms:
    1. Enrollment Form, all students
    2. Home Language Survey, all students
    3. Certificate of Vision Screening, grades K-3
    4. Dental Screening, grades K-9
    5. Transportation Information, please see Transportation web page
    6. Google Apps Student Permission, grades 3-5
    7. Elementary Annual Health Review, grades PreK-5
    8. Secondary Annual Health Review, grades 6-12
    9. Digital Communications Policy, all parents must read – no signature required
    10. Migrant Education Parent Form, all students as appropriate
    11. 2016-17 Iowa Eligibility Application, if applicable
    12. 2017-18 Diet Modification Form, if applicable.
  5. Once all forms are complete and you have collected the necessary documents, please send to the enrollment office via email (enrollment@johnston.k12.ia.us) or fax (515-278-6303). You can also schedule an appointment with the district’s Enrollment Specialist by calling 515-278-0470, ext. 1649. You will receive an email indicating the paperwork’s disposition. When approved, you will receive a call from the school building you are assigned to regarding the next steps.

What are the next steps after completion and approval?

  1. Wait to hear from the school building you are assigned to, generally within 48 hours. The Enrollment Specialist will notify building secretaries of all new students.
    1. Elementary Students (Grades K-5)
      The building will assign the student to a classroom and give the parent/guardian a call with this information prior to the student’s first day. If you want a tour of the building, please coordinate this with the elementary office.
    2. Secondary Students (Grades 6-12)
      Secondary students will need to meet with their school counselor to determine the appropriate class schedule before attending classes. The guidance secretary will contact you for an appointment time. All high school students (grades 9-12) will need to have an unofficial transcript with them at the appointment. This helps the guidance counselor create a schedule and determine credits.
  2. You will be emailed your Infinite Campus portal login information. Sign in and pay the building-specific school fees through RevTrak.
  3. You will begin to receive mailings from the district. Please open and read all information sent so you and your student(s) are well-informed.
